FitSystems Inc. agrees to service the Customer's equipment under the following terms and conditions:
SERVICE
The scheduled basic maintenance service provided by FitSystems Inc. includes inspecting, cleaning, lubricating, adjusting and calibrating the equipment listed below. The cost for this scheduled maintenance service is $85.00 per hour including travel time. The cost for the parts is extra unless covered by the manufacturer's warranty. Please note: Additional labor charges will be billed by the half hour, at the same rate of $85.00 per hour. Parts may not be available for some brands of fitness equipment.

APPROVAL FOR ADDITIONAL EQUIPMENT REPAIRS AND PARTS
The Customer agrees to pay for all parts and additional labour to repair equipment not covered under the basic maintenance service charge. During scheduled maintenance service, the Customer hereby authorizes FitSystems Inc. to perform additional services and provide parts to a value not to exceed $ without additional authorization from the Customer. Additional services in excess of this amount will only be provided after authorization from the Customer or their appointed representative or agent has been received by FitSystems Inc.
TRAVEL
A travel charge will be applied to service performed outside the Calgary city limits. This rate may vary with gasoline prices.
FREQUENCY AND SCHEDULED SERVICE
FitSystems Inc. shall provide scheduled basic maintenance service to the Customer's equipment every months.

PAYMENT
Payment will be due within 30 days upon invoicing after each service visit. The service technician will leave a detailed description of services performed, which will accompany the invoice.
TERM AND TERMINATION
The term of this agreement is ongoing from the date of the first maintenance service. This agreement may be cancelled by either party without cause upon 30 days written notice delivered to the other party at the addresses listed above.
